The photo you have linked is not terribly clear, but it kind of looks similar to the Taxan monitor listed in this auction (see the 1st pic):

I know Taxan made a number of models for the Apple II in the early 80's (I have a green screen Taxan from my childhood still in use), so if this isn't the same as the monitor you remember, it might be one of the other Taxan models.
