Well, I have sucessfully dumped all the ROMs for the cards below and made a copy of the 077-0130 and 077-0131 ROMs for the diagnostics card I replicated.
And lo and behold, the ROMs (that I didn't fine official Apple Part Numbers for - and now I know why) are logic board testing the Apple ][ and Apple ][+ units. I didn't dig out an Apple II+ or one of my Apple IIs to see if this card actually works in them, but it looks like it might.
I wanted to post this, but now I am going to go try it and I will post back on this thread what I find out.

To be specific, the ][ / ][+ Diagnostics ROM, is what's needed.

Yes. It appears to be the case. The 077-0130 and 0131 ROMs were never released to the dealers. I suspect these were for Apple Internal Use. I will upload these ROMs somewhere this weekend and post the URL here and on Facebook A2 Enthusists group. I am slammed until the weekend. Also I want to test in my ][ and ][+.

These cards were however, occasionally provided to Apple ][+ developers, although that may have been elicit at the time. I clearly remember one of these in our office. This is the card, and the ROM set, that I remember fro 1981/2. I knew that a diag ROM existed, but until now, I could do nothing other that state that I remember them. I greatly appreciate you finding one, and any efforts that you make to allow public access to it.

Slow down there. We do not yet know what address lines these are using. I may be able to plot it from the photos by visually following traces, but the easiest thing to do ios to test these as the lowest ROM space in an emulator first, to see if it will boot from that range.
An alternative may be to build an Apple ROM card variant, and put EPROMs on it, then see if it can run from a specific address space on that card.
The Apple //e Diagnostic card uses 2764 EPROMs. No adapter needed.
Correct. 2764 or 28C64 EEPROMs will also work.
Aren't these ][+ Diag ROMs, though? Now, they are likely still fine on 2764s, but as far as I could tell, that dump was for the Apple ][ factory test board, not the dealer diagnostics //e card.

I think I put both the 2716 ROMS for the final test card and the 2764 for the diagnostic card up there. Let me double check tomorrow.
