I've got a NuBus version of the MediaVision Pro Audio Spectrum 16. This item is explained well right here on Applefritter:

The unit pictured in Tom Owad's writeup is the LC-PDS version. Mine is the NuBus version, for most Quadras and for NuBus Power Macs. Also, mine doesn't come with the cheesy headphones.
Otherwise mine is complete, in the original packaging. Here's a pic of the box:
