It could be 341-0271-A (5 Meg profile card) or 341-0299-B (10 Meg profile card) or both

I will also upload it on Asimov.
It triggers a question : will the 10 Mo interface work with a 5 Mo Profile ?

As the poster said in #4, the EPROM image is available on Asimov, see here.
Both the 5MB and 10MB ProFile controller images are also available at the Apple II Documentation Project.
Note that these are for the interface card installed inside the Apple II; the board located inside the ProFile also has a ROM piggy-backed on its Zilog Z8 microcontroller, which is different. An image for the 5MB ProFile is also on the Apple II Documentation Project site.
